第一節 施工現場質量管理檢查記錄表
該表是GB50300—2001第3．0．l條的附表，健全的質量管理體系的具體要求。一般一個標段或一個單位（子單位）工程檢查一次，在開工前檢查，由施工單位現場負責人填寫，由監理單位的總監理工程師（建設單位項目負責人）驗收。下面分三個部分來說明填表要求和填寫方法。
一、表頭部分
填寫參與工程建設各方責任主體的概況。由施工單位的現場負責人填寫。工程名稱欄。應填寫工程名稱的全稱，與臺同或招投標文件中的工程名稱一致。
施工許可證（開工證X填寫當地建設行政主管部門批準發給的施工許可證（開工證）的編號。建設單位欄填寫合同文件中的甲方，單位名稱也應寫全稱，與合同簽章上的單位名稱相同。建設單位項目負責人欄，應填合同書上簽字人或簽字人以文字形式委托的代表一工程的項目負責人。工程完工后竣工驗收備案表中的單位項目負責人應與此一致。
設計一單位欄填寫設計合同中簽章單位的名稱，其全稱應與印章上的名稱一致。設計單位的項目負責人欄，應是設計合同書簽字人或簽字人以文字形式委托的該項目負責人，工程完工后竣工驗收備案表中的單位項目負責人也應與此一致。
監理單位欄填寫單位全稱，應與合同或協議書中的名稱一致。總監理工程師欄應是合同或協議書中明確的項目監理負責人，也可以是監理單位以文件形式明確的該項目監理負責人，必須有監理工程師任職資格證書，專業要對口。
施工單位欄填寫施工合同中簽章單位的全稱，與簽章上的名稱一致。項目經理欄、項目技術負責人欄與合同中明確的項目經理。項目技術負責人一致。
表頭部分可統一填寫，不需具休人員簽名，只是明確了負責人的地位。
二、檢查項目部分
填寫各項檢查項目文件的名稱或編號，并將文件（復印件或原件）附在表的后面供檢查，檢查后應將文件歸還。
l、現場質量管理制度。主要是圖紙會審。設計交底、技術交底。施工組織設計編制審批程序。工序交接、質量檢查評定制度，質量好的獎勵及達不到質量要求處罰辦法，以及質量例會制度及質量問題處理制度等。

Design a unit filling out the design contract signature the name, its full name should be consistent with the seal on the name. Design units of the project leader column, the design contract should be signed or the signatory in writing which commissioned the project leader, after completion of final acceptance in the table of units the project leader should also be consistent with this.
Supervision units filling out the unit name, should be the name of the contract or agreement in line. Column should be the chief supervision engineer contract or agreement clearly responsible supervision of projects, supervision units can also be clearly documented person in charge of the project supervision, supervision of qualified engineers must have a certificate, professional counterpart.
Construction unit filling out the construction contract signature unit of the name, the name and signature on the agreement. Project Manager column, project technical lead in the specific field and the contract project manager. Project Technical Leader unanimously.
Fill in the header section can be uniform, without a sign off staff, but clearly the responsible person position.
Second, check the portion of the project
Complete the inspection project file name or number, and documents (copy or original) is attached to the back of the table for inspection, inspection of documents should be returned.
