質(zhì)量監(jiān)督機(jī)構(gòu)在交工驗(yàn)收前質(zhì)量檢測(cè)、檢查，竣工驗(yàn)收前復(fù)測(cè)、復(fù)查和監(jiān)督過程中檢查資料基礎(chǔ)上的形成工程質(zhì)量鑒定報(bào)告
1、工程質(zhì)量鑒定報(bào)告注意事項(xiàng)
以竣工驗(yàn)收前的復(fù)測(cè)的資料替代交工驗(yàn)收時(shí)檢測(cè)的相應(yīng)指標(biāo)的數(shù)據(jù)，并對(duì)發(fā)生變化或衰減情況進(jìn)一步分析，在鑒定報(bào)告中進(jìn)行說明。
外觀質(zhì)量也應(yīng)在竣工驗(yàn)收前進(jìn)行復(fù)查，特別是結(jié)構(gòu)物的變形、裂縫、高邊坡的穩(wěn)定情況、不良地質(zhì)路段的路基穩(wěn)定性，路面的病害及修補(bǔ)情況等應(yīng)進(jìn)行詳細(xì)復(fù)查，以全面掌握工程質(zhì)量的現(xiàn)狀。復(fù)查的結(jié)果作為質(zhì)量鑒定的依據(jù)。對(duì)外觀變化較大的部位應(yīng)在鑒定報(bào)告中做出詳細(xì)說明。
交工驗(yàn)收檢測(cè)提出問題的落實(shí)情況，交工驗(yàn)收證書提出問題的落實(shí)情況也應(yīng)作為復(fù)測(cè)的主要內(nèi)容，在鑒定報(bào)告中做出說明。
